Registering as a new patient
To register with our practice, patients who live within our practice boundary area (W4 and TW8) are asked to do the following:
Bring in some form of identification, with proof of residency at that address such as a driving licence or a document with your name and address on it or a current medical card
Complete a GMS1 registration form. This can be downloaded off the internet by clicking here, and can be found in your 'welcome pack'
You will be given a 'welcome pack' on registering at the practice and will need to then make an appointment for a 'new patient health check'. Please ask the receptionist to book this check
Included in your welcome pack is a urine bottle – please bring a specimen of urine with you to your appointment so that we can test for diabetes and kidney function.
